Reception and Registration
How to find us — We are located at 347 Andrieux Street, in Sonoma, two blocks off of Highway 12 (Broadway). See this map or call us at 707-935-5000 for directions.
What you will need for admission — Our admitting representative will ask about your insurance coverage. You will need:
- Insurance identification or policy number
- Social Security number
- Medicare/Medi-Cal card
- Any other medical insurance information your doctor suggests you bring
- If you have an Advance Directive (Living Will/Power of Attorney for Healthcare), please bring a copy of it with you.
You should allow approximately 15-30 minutes to complete the admissions process.
- You can pre-register for lab and radiology procedures saving you time during your visit to the hospital. Please call 935-5025 for further information.
- We will ask to collect any deductible, share of cost and/or copay at the time of registration. We will also ask for a deposit if you do not currently have insurance.
- Sonoma Valley Hospital is pleased to offer the services of our Financial Counselor should you need to establish a payment plan, apply for Medi-cal or would like information regarding our generous financial assistance program. Please call 935-5021 for further information.
What will you need for your stay? — We will provide you with most of what you need for your hospital stay. Please bring a list of the medications you are currently taking, but do not bring your medications or use these medications while you are in the hospital unless you are told to do so by your physician.
- Personal Appliances—Insurance requirements dictate that any privately owned electrical items that you bring with you (such as personal hair dryers, shavers, radios, etc.) must be inspected by our Engineering Department. Check with your nurse to arrange for inspection.
- Smoking—State law and concerns for patient safety mandate that smoking not be allowed by anyone within the hospital building. There are several areas designated for that purpose outside the building. Consult your admission materials for more information.
- Telephone service is available in your room from 7 a.m. to 10 p.m.
- Cable television is included in all patient rooms.
- We encourage you to leave your valuables at home. Bring only what you need to make your stay as comfortable and stress-free as possible
